Abstract: This study evaluates the safety and efficacy of intravaginal low dose (25µg) of Misoprostol for cervical ripening and labour induction in primigravida with term gestation. Two hundred and forty seven patients with an indication for induction of labour in third trimester were included. The cervical score was assessed by using Bishop’s score prior to prostaglandin induction. Patient’s with Bishop’s score <4 were induced with misoprostol (25µg) 4th hourly maximum of 4 doses. Data’s were analyzed with student ‘t’ test and P-value of >0.05 were considered not significant. Misoprostol was significantly more effective in inducing labour (98%) with the number of failed induction being 5 out of 247. 94% of Patients delivered within 24 hours. Mean time from start to delivery (including caesarean) was 12.97+6.34hours. Lesser number of patients required oxytocin augmentation i.e. 32%. Induction to vaginal delivery interval was 11.9+4.9 hours. Induction to vaginal delivery interval was significantly greater with Bishop’s score <3 i.e. 12.53+5.7 hours compared to Bishop’s score >3 i.e. 10.2+3.7 hours. 97.56% of patients delivered vaginally within 24 hours. 68.29% of patients delivered vaginally with single dose of Misoprostol with mean number of dose 1.34+0.52. Dystocia, non reassuring FHR with hyper stimulation and tachysystole were 2%, 4% and 4% respectively. Neonates with Apgar score of <7 at 1 minute and 5 minutes were 12% and 2% respectively. The incidence of thick meconium and neonatal intensive care unit admission being 14% & 12% respectively. Intravaginal low dose Misoprostol is effective and safe drug for induction of labour.

Abstract: Asymptomatic pyuria among diabetics is a condition which is largely undiagnosed and hence ignored in India. A community based cross sectional study was carried out to measure the prevalence of asymptomatic pyuria among diabetics in Vellore district of Tamilnadu. A total of 117 study participants were selected through systematic random sampling method. Among all diabetic patients, 26.5% were found to have asymptomatic pyuria who were either nitrite or leukocyte esterase positive. It was also noticed that more than 20% were males which suggest that not only females but males were also at a risk of getting asymptomatic pyuria, possibly related to old age.

Abstract: Histopathological diagnosis of benign prostatic hyperplasia (BPH) and prostatic carcinoma (PC) may be problematic. P63, is confined to basal cells/myoepithelial cells in prostate. Cyclin D1 is expressed in the G1 phase of cell cycle and play important role in regulating the cell cycle and cancer progression and Its over-expression is believed to play a role in tumorigenesis including prostatic carcinoma.

Objectives: This study is to assess the expression of P63 and cyclin D1 in BPH and PC, to examine the correlation between results of expression P63 and cyclin D in such lesions, determine the relation between the immunostaining and histologic grade, stage of PC as well as clinical and radiologic findings.

Material and methods: 50 cases of BPH and 50 cases of PC were obtained by TURP (62 cases) and radical prostatectomy (38 cases). For each case, clinical data and radiographic findings were obtained. All immunohistochemical (IHC) analysis was performed on routinely processed, formalin-fixed, paraffin embedded tissue. Tissue sections were cut at 4 µ and mounted on poly-L-lysine-coated slides. Percentage of positive cells was calculated and positive staining scored as: 1+ (weak)= less than 10%, 2+ (moderate) = 11 to 50% and 3+ (strong) = more than 50% tumor cells stained positive.

Results:  For P63; 98% of BPH showed positivity and 96% of PC cases showed negativity. For Cyclin D1; 84% of BPH showed negativity while 90% revealed positivity. Degree of reactivity was increased with high Gleason grade but this correlation is not significant.

Conclusion: p63 and Cyclin D1 were highly expressed in BPH and PC respectively, so they may be a valuable tool in differential diagnosis of BPH versus PC lesions.

Abstract: Transvaginal sacrospinous colpopexy is a surgical technique applied to suspend the vaginal vault from the sacrospinous ligament to prevent vaginal vault prolapse. We herein present our experience in applying this technique in twenty women with either marked uterovaginal prolapsed or vault prolapsed.  All patients were followed up over a six month period. We met with no intraoperative complications during the procedure. Postoperative complications such as fever (4.8%), urinary tract infections (23.9%), lower back and gluteal pain (14.3%) and urinary retention (14.3%) were recorded. None of our patients returned with recurrence of prolapsed during the flow up period of six months. Sacrospinous colpopexy is a simple, effective and safe technique that can be performed either by itself or as an adjunct to vaginal hysterectomy.

Abstract: The dual-energy computed tomographic (CT) technique allows the differentiation of materials with large atomic numbers such as iodine and xenon. The basic principle of dual-energy CT is material decomposition based on attenuation differences at different energy levels. By using dual-energy CT angiography for the evaluation of perfusion defects in cases of pulmonary embolism, and using xenon CT for the evaluation of ventilation defect.

Abstract:  People are exposed to a variety of potentially harmful agents in the air they breathe, the liquid they drink, the food they eat, the surface they touch and the products they use. An important aspect of public health is the prevention or reduction of exposures to environmental agents that contribute, either directly or indirectly, to increased rate of death, disease, discomfort or disability.

The objectives of this study are to measure the quality of biological agents i.e. bacteria and fungi in air and dust at Shendi town and to show the types and the presence of the allergens in the area.

The study was conducted by environmental monitoring, questionnaire and time - activity surveys. The town has been divided into five sectors, from each sector 24 air samples were collected randomly by multistage sampling technique; the total is 180 samples.

All project's data are stored in Microsoft Excel spreadsheets. The preliminary characteristic and indoor pollutant exposure range also were calculated using Excel. Data were also exported to SPSS for additional statistical analyses. Descriptive analyses included frequencies, means and standard deviations as well as analysis of variance for mean comparisons and analyses for categorical variables.

The total exposed Petri dishes for bacteria growth were 120. Most of them were Gram –ve baclli 13 species. The most abundant frequent species were Pseudomonos aeraginosa, Hafnia alvei, E.coli, Cedecea davicae, Providincia rettgeri and Klebsiella pneumoniae. Of the total 120 exposed Petri dishes fungal spores were collected on only 105.The colonies were identified up to the specific level into 17 species. The most abundant frequent species were Aspergiluis, Penecillium, Alternaria; Cladospormm; Curvularia; Fusarium. Thus a comprehensive study, of indoor air biological species, should be done by the Sudanese Standards &Metrology Organization (SSMO) in order to establish Standards for biological allergens.

Abstract: Estimating prevalence of breastfeeding cessation among mothers in reproductive age .and estimating knowledge attitude of mother towered breastfeeding. provide health education into intervention. Compare between control and interval sample.

Randomized interventional trial (preintervention phase –interventional phase –postinterventional phase). The study was applied in Saudi Arabia, AL-Qassim, buraidah2011-2012 in MCH. A self –administrated questionnaire was used to collect data.

More than 49.1% of participating women lay between age group (26-35). 75.4% of the sample was multi pares. Over than 70.2% are have normal delivery.

About 4.44% has to stop breastfeeding in force of the father but it decrease to 2.22%.. Mothers believe that Breastfeeding and formula has the same benefits by mean score equal 4.62 in interventional group which slightly increase after the massage to 4.68.

While the mean core for who said that the formula can’t be as much as Breastfeeding benefits 4.82. And become 4.84 after the massage. 46,6% of the mothers didn’t take the support and take help to learn how to breast feed her child but the percentage for asking help increase to 71.1% after the massage. Also mothers was plain to breast feed the next child by 22% which markedly change after the massage to rich 48%.

Mothers in both intervention  and control group had  satisfactory knowledge level  considering breast feeding benefits for mothers and child, so shortage of knowledge was not the  main cause for decrease breast feeding practice among them breastfeeding wasn't the first choice for the mothers during the first months due to many  precipitating factors as example : it is painful,  mothers taking drugs,  baby was sick, father ask  her to stop lactation , or she is worker mother and there is no nursery  in the work place, but after the health education massage and discussion with mothers in intervention group the mothers attitude toward breast feeding practice was improved as they learned how to overcome these difficulties.

Most of the mother didn't take the support and learn how to breast feed her child from the doctor or nurse which may be the cause of some mothers decided to stop breast feeding without consultation.

Abstract: An association of psychological stress with atopic dermatitis is widely accepted. Serious skin diseases, particularly atopic dermatitis, have a negative effect on patients’ total wellbeing. Patients often experience significant psychological and social distress such as a higher level of depression and fear of judgment. Skin illnesses can also affect patients’ work-related lives by causing them to miss work or be less productive. High top quality of lifestyle instruments provide important info for doctors, the public, and those involved in submission of medical care resources, which inhibits chronic epidermis condition from being overlooked among other health conditions. Abstract: Left ventricular hypertrophy(LVH) is maladaptive response and risk factor in chronic pressure overload. Increased myocardium results in diastolic and/or systolic failure, atrial fibrillation and sudden death. As hypertrophic growth  slow and clinically invisible, early diagnosis is essential to control the mortality and morbidity. Literature Survey on proposed method could not fetch any search results on Google Scholar.

Study aims at establishing indices for diagnosis of LVH by using safe and non invasive techniques and to indicate modification in autonomous nervous system in hypertensive subjects.

Methods and subjects: Conformative diagnostic indices obtained from 2-lead ECG sample collected for 3-5 minutes duration. Novel algorithm computs an index That  provides a guideline to the echo-cardiologist to conduct echo cardiographic study. 

Study was carried out with 26 normal subjects varying in age.sex and economic class, 25 hypertensive and LVH subjects and 27 hypertensives. All the cases were recorded at Fortis-S.L.Raheja hospital Mahim(W).

Essential Results: The values of heart rate, heart rate variability, sympathetic power, total power are significantly higher for hypertensive patients with LVH than the hypertensive and control group.

Principle conclusion: Early diagnosis of LVH by ECG signal Is possible. The p-value was found to be in the range below 0.001.

Abstract: This brief article touches upon and discusses some newer areas of interest in thermoregulatory mechanisms operating in the human system. The male gonad can best function optimally only when it’s ambient temperature is lower by a few degrees than the core body temperature. We present here tow such cooling mechanisms – one the chaotic system and the other the surface area increase theory due to differing levels of suspension to the two testicles.

Abstract: Separation of any kind of protein from a complex mixture can be achieved by numerous methods. In this research experiment, SDS-PAGE technique was used. Samples of H2B and H3.3 protein were loaded in calculated manner and band images obtained after 1D electrophoresis were analyzed. Biomarker was taken as a reference to explore the migratory pathway followed by the sample protein. Basically, the impact of Bisphenol A (BPA) on protein in stimulated human cancer cells was governed. It was concluded that BPA induce mutation in histone protein by altering its molecular mass which depicts a correlation between BPA and cancerous cells.

Abstract: Blood transfusion safety begins with healthy donors. Blood Donors are deferred for multiple reasons and are not able to donate blood successfully either permanently or temporarily.

Aim is to study the main causes of pre-donation deferral of potentially healthy prospective  blood donors in a Teritiary care teaching hospital,  Blood Bank unit,

A retrospective analysis of records of the donors  from January 2014 to December 2014 was done, in order to find out the rate and causes of deferral in pre-donation period  both in male and female donors, in Teritiary care teaching hospital,  Blood Bank unit,

Results Of the 1548 donors registered for donation, deferral donors constituted 114 for various reasons and deferral rate was 7.3%.  The three most common reasons for deferral were low hemoglobin levels, low body weight, hypertension and alcohol consumption. The deferral rate was higher in the age group of 18-25 years and most common cause was under weight, low hemoglobin level. In 45-55 age group reasons for deferral were hypertension, alcohol consumption and low hemoglobin levels. The most common cause of deferral in our study and in several studies available in the literature are the same.

Out of 114 deferred donors males constituted 89.47% and  females were 10.52%. The percentage of deferred donors was 7.3% with majority of them (92.98%) being deferred for temporary reasons. Anemia was the most frequent cause for donor rejection (32.8%). Permanent deferral accounted for 7.01% with hypertension being the most common cause (25%) in this category.

Abstract: The effect of these cardiovascular diseases on the natural history of the underlying liver disease is considered. Their recognition and management is important in the long term care of patients with chronic liver disease, (Maisaia et al., 2001). Almost one third of cirrhotic patients can be shown to have evidence for a cardiomyopathy.  The Q-T interval is prolonged in a substantial fraction of patients with cirrhosis, thus indicating delayed repolarization. However, no information is available in portal hypertensive patients. More specifically, QTc-interval prolongation has been found in association with both alcoholic and nonalcoholic liver disease.

Objective is to evaluate any alternative changes in Q-T interval in portal hypertensive cirrhotic patients.

Patients presented to our clinics at Salman Bin Abdul Aziz University hospital with any of any manifestations of hepatic cirrhosis such as liver stigmata (spider nevi, palmer erythema, gynecomastia, duptryen contracture) with compensated or decompensated liver (ascites, hypoalbumeniemia, hyperbilirubinemia etc..) were enrolled from the Salman Bin Abdel Aziz University Hospital, Al Kharj, Saudi Arabia at that period between 2012-2013. Patients responded to a questionnaire to investigate possible liver cell failure, and then underwent.  

This study was rolled on 80 patients and healthy as a control, 30 patients with early cirrhotic patients without portal hypertension (group II), compared with 30 patients with radiologically confirmed portal hypertension on top of cirrhosis (group III) and 20 healthy persons (group I).

The studied groups were subjected to thorough clinical assessment, routine laboratory investigations, ECG and radiological imaging assessments by: abdominal ultrasonography, Doppler and colored duplex. Doppler   and   duplex examination showed significant differences between normal persons, cirrhotic & patients of portosystemic anastomosis in all values of the portal vein.

There was a significant difference between normal persons, cirrhotic & patients of portosystemic anastomosis in all values of Q-T interval. The Q-T dispersion was correlated with the PV (A), PV (V), PV (F) and CI, in patients of group III.

The delayed repolarization of the myocardium already occurs in cirrhotic patients with or without increase in portal pressure but, it is related mainly to the presence of portal hypertension.

Abstract: Post operative wound infection is the commonest wound infection. It occurs if the integrity and protective function of the skin is breached. To identify the bacterial pathogens responsible for postoperative wound infections and determine their antibiotic sensitivity pattern. A total of 200 pus samples collected from surgical sites and immediately inoculated on blood agar and Mac conkey agar plate’s .Then the culture plates were inoculated at 37 0c for 24 hours. After incubation, isolates were identified by using gram staining and biochemical reactions. Sensitivity tests were performed on Muller-Hinton agar plate by Kirby-Bauer’s disc diffusion technique. This retrospective study was conducted over a period of six months from June 2014 to December 2014, at Narayana medical college and General hospital, Nellore. During this period, a total of 200 samples were collected, among these 136 (68%) showed positive growth. The most frequent isolate was staphylococcus aureus 38 (27.9%) followed by pseudomonas 28 (20.5%), E. coli 26 (19.1%). Antibiotic sensitivity test of the isolates showed that Ampicillin + sulbactam (100%) and vancomycin (95%) were the most effective antibiotics for gram positive bacteria and least effective for ofloxacin (25%). Gram negative isolates were E.coli (100%) , klebsiella (80%), citrobacter (60%), Acinetobacter(60%) were most sensitive to imipenum. Gram negative isolates were E.coli (60%) followed by klebsiella (80%), citrobacter (50%) and Acinetobacter (100%) were highly resistant to co-trimoxazole. Therefore antibiotic policy may help in the prevention and treatment of multidrug resistant pathogens in wound infection.

Abstract: Coronary artery disease (CAD) is a multifactorial disease with no known definite cure, but preventable and treatable.  Many studies have linked excess generation of reactive oxygen species (ROS) with cellular damage and CAD.  No systematic studies were conducted to evaluate the role of oxidative stress and antioxidant status on CAD severity. Hence we have undertaken this study. One hundred and twenty four angiographically proved CAD patients were included in this study. CAD severity was assessed by angiography. The patients with 0 vessel disease were taken as control to study the correlation of lipid peroxidation with CAD severity. Lipid peroxidation product, Malondialdehyde (MDA), antioxidants such as Super Oxide Dismutase (SOD), Glutathione (GSH) and Ascorbic acid of all patients were estimated in the serum. Our results revealed that there was higher lipid peroxidation with CAD severity as indicated from lower values of  MDA  and higher values of SOD in 0 vessel disease compared to single vessel, double vessel and triple vessel disease patients (p<0.05). The severity of CAD is closely related to increased lipid peroxidation and decreased antioxidants status as indicated by the MDA and SOD levels.
